Assessment Process
The Evaluation Process involves the following steps:
Initial contact: We will begin the process with a 30 minute conversation to discuss your questions and what brings you to inquire about testing. This will inform the type of evaluation needed to address your concerns and fees will be discussed at that time.
Scheduling and Forms: If testing is to proceed, we will schedule appointments for an interview, testing, and feedback. Currently, I have returned to in-person testing, though aspects of the assessment may be conducted virtually if needed. I will send electronic versions of history forms and consents for treatment that will be returned electronically before our scheduled interview.
Interview: The aim of this one-hour interview is to collect relevant information on history, current challenges, and your goals for the evaluation.
Testing: Each evaluation is tailored to your specific set of questions and challenges and the amount of time needed may vary. However, testing sessions are typically 6 hours. This can be broken into two sessions if needed.
Feedback: Two weeks following the conclusion of testing, we will meet for one hour to discuss the findings. This is an oral feedback. A written report will be sent to you approximately two weeks following this feedback meeting.
Fees: Specific fees will be discussed in the initial contact phone call once I assess what assessment modalities will be needed. I only accept direct payment and do not accept insurance. Fifty-percent of the fee is due at the time of the initial interview and the other 50% is due at feedback.
You can submit to your insurance company for reimbursement. Should you want to seek reimbursement from your insurance company for a portion of your out of pocket payment for the evaluation services, I can provide you with an invoice that includes all of the relevant information (CPT and diagnostic codes, license and NPI numbers) that you will need to submit to your insurance company. Please be advised that my services will be considered out-of-network. I am not able to intervene or work directly with the insurance company. I recommend that you notify your insurance company in advance to explore the likelihood of your being reimbursed for the testing. You may be able to obtain authorization to be assessed on an out-of-network basis. You will need to contact a customer service representative and request such an authorization. Also please be advised that many insurance companies do not cover testing services for learning and attentional disorders or where the goal of the testing is for educational purposes.
